Both PVC laryngeal masks and
silicone laryngeal masks are supraglottic airway devices (SADs) with identical clinical functions: they are positioned in the pharynx to seal the esophagus and facilitate ventilation during general anesthesia or emergency airway management. While most are disposable, some silicone versions can be sterilized and reused.
Similarities
1. Basic structural design: Both feature an airway tube, an inflatable cuff, a pilot balloon, a one-way inflation valve, and a connector; the gel laryngeal mask is essentially a silicone mask with a modified cuff surface.
2. Core function: Both establish a non-invasive artificial airway to avoid tracheal intubation and are used for anesthesia during minor surgeries, difficult airway management, and emergency resuscitation.
3. Size classification: Both use sizes 1 through 5, selected based on patient weight.
4. General safety requirements: Both must comply with ISO 10993 standards for biocompatibility (non-cytotoxic and non-sensitizing) and share similar risks regarding air leakage, aspiration, and laryngeal injury.
Differences
1. Material Properties
PVC (Polyvinyl Chloride; typically plasticized PVC)
- Relatively high hardness; remains rigid at room temperature but hardens in the cold and softens as the temperature rises.
- Contains plasticizers (DEHP or alternatives); regulatory limits apply to the amount of leaching. Cannot withstand high-temperature, high-pressure sterilization (deformation and increased leaching occur at 121°C); suitable only for Ethylene Oxide (EtO) sterilization and single-use applications.
- Low cost; inexpensive raw materials.
Medical-grade Liquid Silicone Rubber (LSR) / Solid Silicone
- Excellent elasticity; soft with good resilience; superior thermal stability (does not soften significantly at body temperature).
- Free of plasticizers; withstands high temperature and pressure (134°C); some models are designed for reuse (up to 30 times, per manufacturer specifications), though the vast majority are supplied as sterile, single-use devices.
- Higher costs for raw materials and molds; higher selling price.
2. Cuff Tactile Feel and Sealing Performance
- PVC Cuff:
Moderate elasticity; typically has thicker walls. It becomes slightly more rigid after inflation and offers poorer conformability to the pharyngeal mucosa; pressure distribution is inferior to silicone, making localized high-pressure points more likely.
Advantages: Controllable deformation; mature dip-molding manufacturing process; high production yields are easily achieved.
- Silicone Cuff:
Extremely soft with excellent conformability; it adapts better to the irregular contours of the pharynx and provides a seal at lower pressures. Theoretically, this results in a lower incidence of pharyngeal mucosal ischemia and postoperative sore throat.
Disadvantages: Silicone is prone to over-inflation, requiring strict control of inflation volume; molds for liquid silicone are expensive.
Gel Laryngeal Mask: Silicone base + hydrogel coating; further reduces friction and postoperative sore throat; represents an upgraded version of the silicone laryngeal mask.
3. Manufacturing Process
PVC Laryngeal Mask
- Airway tube: PVC extrusion;
- Cuff: PVC dip molding (dipping) process—dipping a mandrel into PVC plastisol, followed by heating to gel and form the part;
- Connector and pilot balloon: Injection molding;
Silicone Laryngeal Mask
- Airway tube: Extruded silicone tubing;
- Cuff: Liquid Silicone Rubber (LSR) injection-molded (a few older models use a dip-molding process);
- Component bonding: Silicone heat-press bonding; standard adhesives cannot be used (as they do not bond effectively with silicone and biocompatibility must be considered);
- Mold precision requirements are significantly higher than those for PVC.
- Bonding: Thermal or solvent bonding using PVC-specific adhesive.
4. Clinical Characteristics
PVC Laryngeal Mask
- Advantages: Low cost; suitable for high-volume use, day-case surgery, and resource-limited settings.
- Disadvantages: Relatively stiff; slightly higher incidence of postoperative sore throat in some patients; hardens in low-temperature environments, making insertion difficult.
Silicone Laryngeal Mask
- Advantages: Soft with superior lubricity; smoother insertion; lower incidence of sore throat; temperature-resistant.
- Disadvantages: Higher cost; some products have a high surface friction coefficient and require lubrication.
5. Regulations and Market Trends
- PVC: Still widely used in hospitals globally; however, with the EU tightening controls on DEHP plasticizers, an increasing number of brands are shifting toward DEHP-free PVC or silicone.
- Silicone: Represents the high-end, mainstream upgrade path, with gel-coated silicone at the forefront of innovation.
